Maybe there is another solution, since Illustrator is able to convert from CMYK to Pantone. So, it is possible to: export the InDesign swatch to .ase, import it in an Illustrator file, convert to Pantone, save the swatch as a new .ase file; and import the new .ase file in the InDesign file; replace the CMYK colors with the Pantone colors
